医療施設へのアクセシビリティーと健康指標：日本の高齢者を対象にした研究

Being one of the countries with the longest life expectancy, Japan has implemented a Community-based Integrated Care System (CbICS) as a paradigm that enables aging in place and provides comprehensive support, rehabilitation, and care services to the elderly. Commuting care service facilities are supposed to play an important role in CbICS because they enable the elderly to inhabit their own house and provide community meeting opportunities with their friends. Since 2000, non-government sectors have been allowed to establish new facilities or to operate the facilities that were formerly run by local government. These sectors need to seriously consider the profit for the continuation of the services, which has probably resulted in the more careful selection of the location. Specifically, the influence of potential employees (i.e., the working-age population aged 15-64 years) and potential users (i.e., elderly aged 65 years or older) around the facilities were assumed to be considered. The objectives of the present study are: 1) to evaluate the accessibility to commuting care service facilities for the elderly aged 65 years or older for all the populated 500m grids in Japan, and 2) to investigate the associations between accessibility to commuting care service facilities and demographic variables (i.e., sex-specific number of individuals aged 15-64 years, 65-74 years, and aged 75 years or older, and sex-specific proportion of individuals aged 15-64 years, 65-74 years, and aged 75 years or older ) in each grid, and 3) to investigate the associations between accessibility to commuting care service facilities and health ooutcomes.
